Insurance Networks
I am in-network with the Blue Cross Blue Shield of Illinois PPO and BlueChoice PPO networks. I am happy to submit insurance claims to Blue Cross on your behalf for payment. At the time of your appointment, I may charge any relevant co-pay/co-insurance charge associated with your plan, preferably via check or credit card (Visa, Mastercard, American Express, or Discover).
Please note that I am unable to accept those patients who are covered by Medicare.
Out-of-Pocket Payment
For those patients who prefer not to use insurance, or who are not in my insurance network, I do offer an out-of-pocket fee schedule, as detailed below:
- Initial Consultation (90 minutes)—$350
- Psychotherapy Follow-up (45 minutes)—$225
- Medication Management Follow-up (30 minutes)—$180
